Requirements for participation in NEO Athletics:

1)      Forms are to be printed, read, and completely filled out (link is below)

2)      If you have had any surgery in the last year you must have a

             written release from your doctor.

3)      Current physical (no older than June 1)

             Physical must have the stamp from the physicians office

             Physical must be from a physician or physician assistant

             Physicals done by a chiropractor or nurse are not accepted

4)      Proof of health insurance (front and back copy of card)

          a. Purchasing NEO athletic insurance is strongly recommended

          b. Short Term Insurance Policies will not be accepted

  

  Participation Forms

 

No athlete can participate untill all requirements are fulfilled and the forms are submitted to the head athletic trainer. After they are sumbitted the athlete can be cleared by the Athletic Trainer.
