Requirements for participation in NEO Athletics:

1)      All forms are to be printed,read, and completely filled out (links are below)

2)      If you have had any surgery in the last year you must have a written release from your doctor.

3)      Current physical (no older than July 1)

4)      Proof of health insurance (front and back copy of card)

          a. Purchasing NEO athletic insurance is strongly recommended

No athlete can participate untill all requirements are fulfilled and the forms are submitted to the head athletic trainer. After they are sumbitted the athlete can be cleared by the Athletic Trainer.
